
Your doctor reviewed your DEXA scan and said the words you weren’t expecting: “You have osteopenia.” Your bone density is lower than normal, putting you at increased risk for fractures. Maybe they mentioned medication. Maybe they said to “do some weight bearing exercise.” But what does that actually mean? And can exercise really make a difference?
The answer, backed by substantial research, is yes. Strength training is one of the most powerful interventions for bone health, and it’s never too late to start.

Understanding Osteopenia and Osteoporosis
Bone density exists on a spectrum. Normal bone density means your bones are strong and dense. Osteopenia is the middle ground, where bone density is lower than normal but not yet low enough to be called osteoporosis. Osteoporosis is significantly reduced bone density, with high fracture risk.
The numbers: Bone density is measured by T score on a DEXA scan. Normal is above negative 1.0. Osteopenia is between negative 1.0 and negative 2.5. Osteoporosis is below negative 2.5.
Think of osteopenia as a warning sign. Your bones have weakened, but you have the opportunity to intervene before reaching osteoporosis. And even if you already have osteoporosis, you can still improve bone density and reduce fracture risk.
Who’s at risk? Postmenopausal women are at highest risk due to declining estrogen, which plays a crucial role in bone maintenance. But men can develop osteopenia too, especially as they age. Other risk factors include family history, small body frame, sedentary lifestyle, smoking, excessive alcohol use, and certain medications.
What the Research Says About Exercise and Bone
The evidence supporting exercise for bone health is robust and compelling:
Strength Training Increases Bone Density
A landmark systematic review published in the Journal of Bone and Mineral Research found that resistance training significantly increases bone mineral density at the spine and hip, the two sites most vulnerable to osteoporotic fractures.
Research in the British Journal of Sports Medicine (BJSM) examining exercise interventions for osteoporosis found that progressive resistance training produces the greatest improvements in bone density compared to other exercise types.
A study published in the Journal of the American Medical Association (JAMA) followed postmenopausal women doing high intensity resistance training and found significant improvements in bone density, along with improved muscle strength and balance.
Higher Intensity Produces Better Results
Here’s something important: not all exercise is equally effective for bones. Research consistently shows that higher intensity loading produces greater bone adaptations.
A systematic review in BJSM found that exercises must exceed a certain threshold of intensity to stimulate bone formation. Walking and light exercise, while beneficial for overall health, produce minimal bone density improvements. Higher load exercises like barbell training produce significantly better results.
Research published in the Journal of Strength and Conditioning Research found that programs using heavier weights and compound movements (squats, deadlifts, presses) produced superior bone outcomes compared to light weight, high repetition programs.
Exercise Reduces Fracture Risk Beyond Bone Density
Bone density is only part of the fracture risk equation. Falls cause most osteoporotic fractures, and exercise dramatically reduces fall risk.
A Cochrane Review found that exercise programs reduce fall rates by approximately 23% in older adults. Strength and balance training are particularly effective.
Even if exercise didn’t improve bone density at all (which it does), it would still reduce fracture risk by improving strength, balance, and reaction time. Research in the International Journal of Sports Physical Therapy (IJSPT) has shown that strength training improves multiple fall risk factors simultaneously.

Specificity to High Risk Sites
The spine and hip are the most dangerous sites for osteoporotic fractures. Hip fractures in particular are associated with significant morbidity and mortality in older adults.
Barbell exercises like squats and deadlifts directly load the spine and hips. Research in BJSM has shown that exercises that load specific skeletal sites produce the greatest bone improvements at those sites.
Scalable for Any Fitness Level
Many people assume barbell training is only for young, fit people. This couldn’t be further from the truth. Barbell movements can be scaled and modified for any fitness level.

Builds Muscle Alongside Bone
Strength training doesn’t just build bone; it builds muscle. This matters because sarcopenia (age related muscle loss) often accompanies osteopenia. Research in JAMA has shown that muscle mass and bone density are closely linked. Building muscle supports bone, and both contribute to reduced fall and fracture risk.

Safety Considerations
Understandably, people with osteopenia or osteoporosis worry about safety. Won’t lifting weights increase fracture risk?
Research actually shows the opposite. When performed with proper technique and appropriate progression, strength training is remarkably safe, even for people with low bone density.
A systematic review in BJSM examining adverse events in exercise trials for osteoporosis found that properly supervised resistance training has very low injury rates. The benefits far outweigh the risks.
Key safety principles we follow:
Proper technique. We teach correct form before adding load. Good technique distributes stress appropriately and protects the spine.
Gradual progression. We increase weight slowly over time, allowing bones, muscles, and connective tissue to adapt.
Avoiding high risk movements. We minimize loaded spinal flexion (bending forward under load) and other movements that could increase fracture risk for those with significant bone loss.
Communication with your medical team. If you have osteoporosis or other health considerations, we encourage communication with your physician. For patients who need more individualized attention, our physical therapy team can provide one on one care before transitioning to group classes.
